We Continue Supporting You Long After Your Appointment
Our relationship with patients doesn't end when the eye examination is over. If we identify an eye condition requiring further investigation, we arrange referrals to your GP or Hospital Eye Service promptly — managing the process and keeping you informed throughout.
Our aftercare service includes spectacle adjustments, repairs, prescription reviews and ongoing monitoring of conditions such as glaucoma, cataracts, macular degeneration and diabetic eye disease — all delivered at home.
We work closely with GPs, hospital eye services, care home teams, occupational therapists and families to ensure every patient receives joined-up, coordinated care.
Long-Term Eye Condition Management
Many eye conditions require ongoing monitoring rather than a one-off appointment. We provide regular home visits to track changes and ensure timely referrals when needed.
Glaucoma
Regular monitoring of eye pressure and vision, working alongside your hospital eye clinic to support long-term management.
Cataracts
Monitoring cataract progression and arranging referrals for surgery when the time is right.
Macular Degeneration
Regular checks for signs of wet AMD progression and urgent referral to the Hospital Eye Service when needed.
Diabetic Eye Disease
Ongoing eye health monitoring for patients with diabetes, working alongside diabetic eye screening services.
Spectacle Repairs
Free adjustments and minor repairs carried out at home — just call and we'll arrange a convenient visit.
Liaison with Care Teams
We communicate findings to GPs, families, care home managers and other healthcare professionals as required.

We understand that many of our patients are already under the care of multiple healthcare professionals. Our aftercare service is designed to complement and support your existing care — not to replace it.
